To have your correct gender acknowledged in our policy system, and so that we can ensure you are included in future system changes, please check this box above. If this fails, the quickest way to receive my refund is via a direct transfer to a bank account - BSB and Account number held in my name as the policyholder on my policy. EFT details provided in the Description box below will be added on my policy to make any future refunds easier.
