I declare that I am a small business within the meaning of section 259A of the Duties Act (NSW) for the income year in which this insurance is effected or renewed.

I declare that I understand that should a subsequent Audit by the Office of State Revenue decide I am not a small business, that any exempted stamp duty (plus potentially interest and penalty tax) will become immediately payable to the Insurer.

I declare that I am the insured or have authority to make this declaration on behalf of the insured.

I acknowledge that if I'm entitled to a refund on my policy, it'll be processed back onto my original payment card/s.

If this fails, the quickest way to receive my refund is via a direct transfer to a bank account - BSB and Account number held in my name as the policyholder on my policy. EFT details provided in the Description box below will be added on my policy to make any future refunds easier.
